New Companies
Welcome to the Biolabs Community! Here’s some info to help you get set up.
Before you can fully join our lovely community and start work on-site, the following forms need to be completed.

New Company Form for ALL New Companies

Join Marketplace​ and take training.

Submit company point of contact name, email, and phone to Admin Coordinator to setup company account.

Review the Orders & Deliveries page.

Review Policies and submit Acknowledgment Forms.

Office Members should review Office Policies, Emergency Action, and Fire Safety​

Lab Members should review all plans and policies.

Add New Members Form. Find out more about Onboarding along with dates.

24/7 Building Access (contact the Site Coordinator to receive access)
New Company Forms and Info for Lab Companies

New Company Lab Safety Questionnaire (must be filled out before lab work starts and submit to the Lab Manager. This will help build your safety binder and additional training.)

Chem-Bio-Equipment Inventory Template (should have any items you plan to bring onsite or purchase right away. Chemical inventories will be updated quarterly)

Select a Lab Head and submit name to Lab Manager. This is the main contact we will use for issues, alerts, and inspections.

All lab member must apply within 45 days.

Must be transferred from other facilities.

Online applications and payments need to be submitted to the system before arriving for the test.

MediaLab Training Modules (The Lab Manager will send each new member a set of training modules that must be completed prior to lab work starting.)

Equipment Approval Form (all equipment coming onsite must be approved prior to purchasing)

Lab Work Alone Form (required for anyone working outside of business hours)
